We share the report published in Heraldo de Aragón By Marisa de la Rica, researcher and leader of the Nursing Research Group on End-of-Life Processes at IIS Aragón and president of the Spanish Association of Nursing in Palliative Care (AECPAL)
The researcher from the Aragon Health Research Institute (IIS Aragon) Marisa de la Rica reflects in a report published in Heraldo de Aragón on the need to strengthen intermediate care within the healthcare system.
Under the title "Intermediate care, the missing link in our healthcare system"The author analyzes the existing care space between primary care and the acute care hospital, especially relevant for older people with chronic diseases, frailty, functional impairment or palliative care needs.
In the article, De la Rica points out that intermediate care allows for accompanying the transition after hospital admission, adjusting care, preventing readmissions, and detecting early situations such as falls, problems with medication, cognitive impairment, loneliness, or caregiver overload.
The researcher argues that this model should be consolidated as a tool for clinical observation, early detection and continuity of care, with the aim of preserving the autonomy of older people and preventing avoidable situations of dependency.
The report also encourages progress towards a more coordinated healthcare system between the health and social sectors, truly focused on people, their dignity and their quality of life.
