Sarcopenia, the gradual and generalized loss of muscle mass and function with aging, is one of the major health problems in older people, given its high prevalence and enormous clinical and socioeconomic implications. Its diagnosis remains unclear, with physical tests that are difficult to access in clinical practice and thresholds with little consensus. Focusing the analysis on exosomes, a subtype of plasma extracellular vesicles, is a new diagnostic possibility, allowing the analysis of protein biomolecules in an accessible and minimally invasive manner. Furthermore, these biomarkers could also be used for monitoring and evaluation of therapeutic responses in the disease. In this context, our first objective is to characterize specific proteins contained in exosomes of sarcopenic patients, reflecting early changes in the disease, to use them as a biomarker for early diagnosis. In addition, we will analyze how the intervention of physical training modifies these biomarkers, in order to use them to predict the efficacy and response of patients to these treatments, the only ones, together with the protein-rich diet, currently available to delay the disease. For this purpose, plasma exosomes were isolated from sarcopenic patients and healthy controls of similar age and their content was analyzed by proteomics. After statistical analysis, a candidate protein was selected from the proteins with significantly different expression between groups and validated by ELISA, confirming the proteomics result. Our results confirmed the existence of differences in the protein charge of plasma exosomes between healthy and diseased patients, and identify robust candidates as biomarkers of the disease. This study opens the door to the analysis of exosomal proteins as biomarkers in sarcopenia
